"Even when memories fade, love remains"
"Even when memories fade, love remains"
Caregivers give their time, strength, patience, and love—often without recognition and too often without enough support. You Are Seen is MOTH’s promise to notice, honor, and walk alongside caregivers before, during, and after the caregiving journey.
Plain-language guidance on changing care needs, communication, planning, self-care, and navigating difficult transitions—shared with compassion and without judgment.
Help identifying trusted local services, respite options, support groups, community programs, and other next steps based on each caregiver’s circumstances.
Personal acknowledgment, uplifting messages, and meaningful moments of appreciation that remind caregivers: your work matters, your love is visible, and you are not alone.
1. NOMINATE OR CONNECT — Tell MOTH about a caregiver who deserves recognition, or reach out for support for yourself.
2. WE LISTEN — MOTH learns about the caregiving journey, current needs, and the kind of encouragement or connection that would be most meaningful.
3. WE RESPOND AND STAY CONNECTED — We offer recognition, relevant education and referrals, and continued community through caregiving and beyond.
When active caregiving ends, caregivers may face grief, relief, uncertainty, or a changed sense of purpose. MOTH continues to offer connection, remembrance, and a compassionate community for the season that follows.
Who this is for: unpaid family caregivers, friends, neighbors, and community members supporting someone through Alzheimer’s, dementia, memory loss, illness, disability, aging, or another life-changing care journey.
You do not need a formal title or diagnosis to belong here. Whether you are preparing to care, caring today, or rebuilding life afterward, You Are Seen welcomes you.
Call MOTH if you would like help completing the form or want to discuss the caregiver’s story first.
Nominate a caregiver who deserves to be recognized, or connect with Memories of the Heart for education, practical guidance, encouragement, and continued community. Every caregiver’s story matters—and every caregiver deserves to feel seen.
